Meeting the needs of patients now and in the future: how our evolving health care system is responding to the challenge

This event is part of a programme of work led by NHS Providers, the Local Government Association, NHS Clinical Commissioners, and the NHS Confederation, to share the learning from the new care model vanguards.

The Five Year Forward View, published by NHS England in 2014, set out a plan to transform the way care is delivered in England in order to meet the changing needs of the population. It argued that the NHS needs to adapt and evolve to take advantage of new science and technology, and to meet the challenges of an ageing population, increasingly living with complex health issues. The Five Year Forward View also argued that health and care services needed to have more engaged relationships with patients, carers and citizens, and to do much more to promote wellbeing and prevent ill-health.
